Register with us as a New Patient
New Patient Registration
Thank you for choosing to register with us. In order to register you need to live within a certain distance of our practice – you can use our online boundary checker below. If you are a UAL student and wish to register with the practice you must reside in the borough of Lambeth, Lewisham or Southwark.

Please complete all the fields on the registration form and the medical summary. Incomplete forms will delay your registration.
Registrations will be processed within 10 to 24 hours.
After receiving confirmation, you should book an NHS Health Check with us by contacting the surgery.
If you prefer not to register online please complete the following steps:
- Please check our map (above) to ensure you live in the catchment area
- Download, print and complete the NHS GP patient registration form, also available from reception.
- Return your completed registration forms to reception or post to us
- If you don’t have an access to a printer and would like us to post a registration form to you please give us a call on 020 7639 3553
